PREOPERATIVE PSYCHOLOGICAL EVALUATION FOR BARIATRIC SURGERY
The preoperative psychological evaluation is a vital component of the weight loss program and aftercare plan.
Designed to support individuals embarking on their transformative journey, this evaluation is specifically tailored for those pursuing bariatric weight-loss surgery.
Why do you need an evaluation before surgery?
This evaluation can identify your strengths, problematic eating patterns and offer pre-and post operative recommendations to improve the long-term outcome after bariatric surgery.
The following components are included in the psychological evaluation:
Assessment of mood, anxiety, drug & alcohol use
Eating disorder tendencies
History of weight loss efforts, eating habits, triggers to overeating etc.
Life stressors
Mental status exam
Psychiatric history
Formal assessment measures
(When necessary this evaluation can be conducted online.)
SCHEDULE AN APPOINTMENT
After the evaluation, a report will be generated and faxed to the bariatric physician (usually within 24 hours).
Fee is $210.00. Dr. Wieliczko is an out-of-network provider. A receipt is available for patients to pursue reimbursement (out of network benefits) from their health insurance company.
Appointments can be made during the weekday and evening as well as Saturdays if needed.
**All patients are entitled to a Good Faith Estimate for Health Care Items and Services (as stated in the No Surprises Act Law). This estimate of costs for the psychological evaluation will be sent to you, in a timely manner (in accordance with the law), by email or postal service based on patients’ preference.
